I pull the image from LR to Topaz Photo AI, fix it (denoise / face etc). Image looks great but when I bring it back to LR some of my photos have these weird additions that weren’t on the original photo OR during the editing inside Topaz. Any idea?

I have the same phenomenon. But it only happens with raw images.
This is an artifact that appears with one of the Raw Remove Noise models.
Please try switching the Raw Remove Noise model to the other one which should prevent this. If you are using the Strong model, switch to the Normal model or vice versa.
